A very minor thing, could marriage age and the age a character becomes an adult lowered?
Though it may be an exception we see a 15 year old Robb leading armies and an even younger Joffrey ruling, at least for some time, without (listening to)a regent. We also see that girls marry when they "flower" and Sansa marries when she's 13.
Lowering adult age to 15 and marriage age to 12/13 would be more in line with the books.
Also making the switch to Adult portaits to 14 or perhaps even 13 would be good too.

Generally if we're getting that low I'd have to call for the need of higher childbirth death risks based on age and the AI being discouraged from marriage immediately upon reaching age due to this, unless they have certian traits, opinions, or political motives. That said, given how betrothals work in CK2 they're essentially marriages in all but name, since they work just fine for NAPs and alliances plus opinion boosts, just without the consummation.

If it is really that important to you, it can be EASILY changed on your own just by giving the defines.lua file a quick edit as all those things are listed in there and very easily understandable so little risk of error with editing it but still make sure to back it up first before making any changes. I myself made some of those changes when i first started playing(not your EXACT changes but nonetheless...)but i preferred portrait change at 14(like to see how characters actually look before committing to a marriage) Also increased age of when the higher adult portraits change such as middle age to 40, and old to 60 etc. A little sample of how it looks so you can see just how easy the changes are to make... As far as betrothals go, they can be so easily abused in this game. For example, i start as a custom independent High Valyrian lord in Bhorash and a few years in if im lucky enough to have a young child or 2 already and i get attacked by a nearby powerful lord in a slave raid like Yunkai. If i dont think im strong enough to hold them off myself yet i can just look to another nearby Valyrian Lord such as Volantis, Tolos, Elyria or Manterys and if they also have a young child*(or anyone close family member willing to marry mine that will get me a NAP) i can quickly make the betrothal, form an alliance, ask their help in the battle, and then after we fight them off, i can keep them around for a bit and then once i get more powerful, i can easily just break the betrothal and betroth my child to who i truly want them to be with and if the other Lord dont like it(and he wont) then too bad for him, cuz other then a bit of an opinion loss, there is no other repercussions for it and if he wishes to come say something about it, ill gladly show them the end of my sword and depending on my luck to that point, possibly even the teeth of my dragon.

And if a red wedding against me was an option id think twice but as i said, no major repercussions, therefore no problem
