Hope that it's not a problem that this is more of a "suggestion list", but I feel that the individual suggestions are too minor to each warrant their own thread.
General
- Traveling to the ruins of Valyria for swords should be disabled, obviously
- If you have the 'obtain valyrian steel' quest, you should have the decision to travel to Valyria and try to find someone willing to part with their sword
- The procedural First Man houses should have more original names, not sure why every single one is called 'of [capital name]'
The North
- Rogar the Huntsman should have less diplomatically negative traits. As it stands, his vassals hate him and tend to overwhelmingly defect to the Starks in the initial war between the two, making it almost impossible to win/white peace the initial war
- Particularly unsavory Boltons should be able to keep the skins of high-level rivals (particularly Starks) as artifacts
- While Bear Island is under ironborn control, the Mormonts should be a landless house. Even better, if the Starks take Bear Island from the ironborn, they should get an event to grant the island to the Mormonts (purely a flavor event that the AI would choose more often than not)
The Riverlands
- If an Andal adventurer defeats the Mudds, the riverlands should have a chance of shattering
- There should be a series of decisions to (more cheaply) raise Seagard and Riverrun, since they aren't present in the earlier start dates.
The Stormlands/Crownlands
- Justin Massey should get some event troops to help him out in his war with the Durrandons. As it stands, it seems impossible to beat the Stormlands back in the initial war
- During civil wars in the Stormlands, a character whose main title is the high lordship of the Hook should be able to declare independence and gain the titular Kingdom of the Hook
- Togarion Bar Emmon should have a claim on the Hook
- It should be possible for the Celtigars to appear on Claw Isle at some point (we have no idea of even the rough estimate of when they arrived in Westeros, but I don't think that it's impossible that they arrived sometimes during the Andal Invasions)
- If an Andal house takes control of Tarth and has it as their main title, they should be able to change their name and CoA to those of House Tarth (only once per game, so that the game doesn't end up with 10 Houses Tarth) and gain the honorary title of Evenstar
Dorne
- Dornishmen should be able to raid, as they did historically
