Could you add in the Osgreys of leafy Lake? They are mentioned in the dunk and egg novellas. A uncle of Ser Eustace was the head of the house and had at least one daughter. But by the time of the sworn sword the line is extinct.

House Osgrey does exist in the mod. If you start in earlier bookmarks they will be at their full strength and gradually lose more and more land by the time of the D&E novellas, dying out entirely by the main books.
