Currently characters from the correct dynasty can recover their lost VS sword on the VS quests but they can’t reforge their ancestral sword if they obtain a VS by other means. For example colonising provinces or having someone else give them a generic VS sword. I don’t see any real reason why if house Royce, say, came across a VS that didn’t have a name they wouldn’t have the handle reforged to match descriptions of Lamentation and give it the name of their lost sword, after all we know both Ice and Lady Forlorn were the names of ancient swords given to the Valyrian blades by their owners to carry on the name.
I’d suggest a decision like the reforge Ice one where if the sword in question doesn’t exist the correct dynasty can reforge a generic sword into a duplicate of the lost sword
