I don't think the option to slay people in melees should really be present. I might be wrong, but my impression of melees in the books is that they are fought with blunted weapons. People do die, but their deaths are accidents or perhaps some covert murder. Though if someone has a passage in the book contradicting this I'll yield the point. I do recall one of the recent info books mentioning a dozen or more men killed at great melee in the North, but again I'd wager those accidents. Infected wounds and concussions and such. Certainly melees are already more dangerous that jousts.
If nothing else, I think that deliberately killing people in melees should not only gain you the dishonorable trait, but also the known murderer character modifier and said opinion modifier from the host of the melee, the employer of whomever you killed, as well as their spouse/parents/siblings. If Ramsay Snow slays Robin Flint at the Hornwood Melee of 297 AC, then Lyessa Flint should have the option at Lord Eddard's next feast to demand justice from Ramsay Snow or Roose Bolton. Of-course to compensate, characters should probably be weighted against murdering people at melees, if they have the option at all, otherwise poor Ramsay or any other cruel/crazy character will never last long in the North (and that's a bad thing?).

Agreed. Too many NPCs love to deliberately murder during what is essentially a realm festival.

Yeah and the melees and tournaments need to be far less severe in terms of injuries. People losing hands and eyes and legs left and right, then dying of infection.
