I prefer games in which the great families are long-lasting and where one's downfall should be a major thing. While the "dynastic stability" gamerule helps prevent houses from dying out in a regular fashion, all too often I see a lord paramount rebelling against the IT in some hopeless war over some petty dispute and losing their paramountcy since they are labeled traitors, and then said paramountcy being granted to some random knightly house. To make the situation even more silly, the original ruling family frequently retains control over their ancestral castle, so you have situations like the Swyfts ruling over the Westerlands from Corn Keep (or whatever their castle is called) while the Lannisters still control Casterly Rock.
My suggestion would be to add a gamerule that prevents king-tier title revocation through normal means (the 'revoke title' button, or the 'forfeit top-tier title' option in the event at the end of a megawar), instead only being possible through attainder (losing all lands) if the house in question if completely crushed in a civil war (lost control of their capital and significant numerical disadvantage, though having the lord of the house imprisoned would NOT grant an immediate 100% warscore), and even then such a thing would incur significant tyranny and only be used by ruthless, cruel, etc. AI characters. As an alternative for people who don't want to have to fight a long civil war and incur significant tyranny just to replace a troublesome lord-paramount, you could just depose the lord in favor of someone else in their house (e.g. their heir, but possibly other people too) which wouldn't incur tyranny and could be done automatically if you have said lord imprisoned.
