I've browsed and seen many screenshots with what appears to be 3D models of the characters. Are these designed in-game, or are they portraits that can be modified?
I ask because I've done some 3D modeling in the past, may have to download the program and give it a shot if they're portraits and not characters designed in-game.

Both. There is a DLC for CK2 that lets you customise portraits in-game (called Ruler Designer. It's $5.99 CAD, I believe), but to actually make parts for the portraits, you'd need to create a mod for that.
