Not sure if it is possible but is there some way to make the religion opinion maluses vary from religion to religion, it has always struck me as strange that there is the same opinion penalty between followers of the seven and the old gods (who since the end of the andal invasions have a pretty good religious accommodation and happily intermarry without too much problems) as compared to say those same religions and r'hllor, indeed one of stannis' main stumbling blocks in getting support is his religion, whereas you don't have riverlords and manderlys refusing to follow Robb as a heretic or Ned disliking Catelyn or Robert because they pray at a sept not a godswood

Thought there was some mild leniences between the two main branches already? Been awhile but dont remember too much problem, but there are a few modifiers if it's that big an issue. Often times the biggest thing is just different religious morals more than anything. While most religions as far as official doctrine are based in the realm of "Dont be an asshole" the specifics vary and can cause some disagreements. Like say refusing certain food preparation methods. Or believing that something is alright or good while the other religion has a different interpretation(Such as skinchanging). Generally this is the reason for the accepted heresy modifier version between religions, in that they get along but have some fundamental disagreements that weakens the bridges between cultures and nations that same religion was sometimes used for.
That said, disliking is a bit much, but maybe finding them weird or having misunderstandings when talking with each other is more likely. Like for instance the old gods and northmen are big on personal responsibility from what I can tell, while the faith of the seven tends to be more on the bigger picture in the world, demonstrated by the execution traditions. This could lead to some fundamental mentality disagreements on various political matters just due to how they were raised, or in the case of zealots, lead to them believing that their disgraceful or disgusting for breaking fundamental things.
