Here is a list of suggestions:
-Two different titles for the King of the Seven Kingdoms- Lord Protector of the Seven Kingdoms and a Titular King of the Iron Throne (Both are only held by one character, obviously). (Not Necessary imo)
-Lords of Westeros should be directly sworn to the King of the Seven Kingdoms. (To be able to appoint lords as councilors as King of the Seven Kingdoms)
-No "Short Reign" modifier.
-A Ser, or a Master, should be sworn to the lord of their Duchy. (Shoreham should be sworn to Harrenhal, Moat Cailin to Winterfell, etc.)
-Lord Paramounts must only have titular titles (and their landed titles) that represent the region that they are Lord Paramounts of except during mega wars.
-The Iron Throne must have an active effect of increasing the vassal limit of the holder by the number of baronies in Westeros.
-Vassalage of Count tier lords transfer to their High Lords during mega wars (only when they are on the same side with their High Lords).
-Vassalage of High Lords transfer to their Lord Paramounts during mega wars (only when siding with their Lord Paramounts).
-Usurpation of titles should not remove claims.
-Trigger for the mega war system is war against the Seven Kingdoms (No mega war if House Stark and House Lannister wars againt each other while Robert is King).
-Lords, when participating in a mega war, will automatically raise all of their bannermen in support for their liege. (Or at least have the choice to specify the number of men they want to give)
-Lords can switch sides during wars. ("Traitor" opinion modifier for the ones that lord betrayed)
-Branches of a House (House Lannister of Darry) should have their own coat of arms based on their titles. (If you give a second son of House Mudd the Lordship of Godsfort, then that branch would have their own coat of arms with the COA of House Mudd and the COA of Godsfort ala Lannister-Darry style, except when it is a cadet dynasty of a bastard)
-Only the main line of a House would use the main COA of the House. (Other members will have the right to bear the COA unless they are landed)
-After a lord or king dies and their son inherits, the remaining children will lose their claim, unless the current lord is childless. (So if Lord Darry had a 3 sons, when the first inherits, the others will have a claim until a son is produced by the first.)
-Revamp the Riverlands. (The layout of the Riverlands, so the Stone Mill should be near Riverrun, I think)
-If possible, a siege should function like this. (If you siege King's Landing, you siege the gates first, then the city, then the Red Keep)
-Citadel should be sworn to the Iron Throne. (Except during wars, when they are independent)
-Claimants to the throne should have the option to form their own Kingsguard. (Renly would have the Rainbow Guard sworn to him, Stannis would have none)
-Courtiers with a trait of at least "Poor Fighter" should be able to fight in wars, and not just sit around like idiots in a court while the rest are dying like flies on the battlefield.
-I know this is a long shot, but naval warfare so that the Iron Isles would have some strength.
-Allies should be able to board your ships. (And you theirs)

-When you raise a vassals levy, it is their commander that leads those men. (And they should lead their divisions when assigned to a superior commander, so if Loras is in charge of the Highgarden levy, and his superior commander is Renly, then the Levy of Highgarden would have Loras as division commander)
-More islands in the Iron Islands, and relocation of Lonely Light. (There were 7 major islands, so that's Pyke, Old Wyk, Great Wyk, Saltcliffe, Orkmont, Harlaw, and Blacktyde, but there were 31 islands in that area, and the Lonely Light is said to be northwest of the main archipelago, with 13 smaller islands around it.)
-More lordships for all of Westeros. (Canon houses, so the map should be revised, I think.)
-If Faith Authority is low, no levy for the High Septon, and no levy at all for the Citadel.
-Negative opinion modifiers against liege should be removed, because the liege is already affected by the negative opinion modifer from traits. (Lady Lothston is cruel, so Lord Vance has negative opinion of her because she is cruel, and again because she is his liege and she is cruel, which I find redundant.)
-Wars only end with the death or imprisonment of the enemy. (I don't think Robb Stark would yield because Tywin controls most of the Riverlands, and neither should he yield, because he is the King in the North, the only king I mean to bow knee to.)
-Higher chance of dying in a battle.
-Only a claimant can wage war for their claim. (I had a playthrough where Dragonstone was awarded by Maekar to some commoner, and House Velaryon fought for the High Lordship of Dragonstone, with their claimant being a 12 year old Daemon Blackfyre. Velaryons won and got kicked out of Driftmark. Now they are High Lord of the Trident, the fools. Oh, and Daemon Blackfyre was assassinated, or suspiciously died, so now it was Aerys, or some adult Blackfyre).
-Certain titles, such as Summerhall, and Dragonstone, should only be awarded to people under certain circumstances. (Dragonstone for the heir to the throne, and Summerhall to a younger son. Both titles must return to the king when the holder dies.)
-Wives of lords adapt that lord's dynasty, and they lose membership to that dynasty if their husband dies and they do not gain the trait Widow. (Barbrey should be a Dustin, not a Ryswell.)
-Lords can only rebel against their liege if the liege is a tyrant. (unless fighting for another claimant)
-Kingdom of Dorne should be Princedom of Dorne.
-No children of the current king can wage a war against their father. (No children of Aegon should wage war against him. I had one with Visenya's son, I named him Jeff, and during his childhood he became my rival because of some toy issue. I think I destroyed his toy, so he destroyed my Kingdom.)
-Negative opinion modifiers against liege should only be when the current lord is not the in the same dynasty as the previous holder. (Unless the previous lord was stripped of his lands by the king.)
