I the newest version, I found that two decisions, Search for a Smith and Go on Foreign Tour, aren't available, despite requirements being met:
Also, I found that I can't find missing Stark children, I tried to do it with console commands but it crashes the game.
And another, when Littlefinger reveals Sansa, she is dead (couldn't replicate to take a screenshot).
