Only people living there, so people for whom it's their capital or only holding. Anyone living elsewhere is safe.

I may be misremembering, but I thought it affected you whether you lived there or not, as long as you hold the county. Not 100% sure, I haven't looked at the event in a while (and can't check from where I'm now) but IIRC it checks for the capital location, so as long as Harrenhall isn't your primary holding you're fine. Like Mantarys.
I was playing yesterday and held Harrenhall for a few years alongside King's Landing and didn't see anything happen to my char.

Turns out I was wrong: the curse is always applied to the current holder (no matter if this is their capital or not), any children of the same dynasty, and any grandchildren of the same dynasty. Even if those children etc. are elsewhere.
