Citizens of Volantis - specifically Old Blood - are allowed to run for office. AFAIK the only requirement is those two things. Therefore known Valyrian houses that can trace their lineages back to Valyria, like Targs, and the other two (three?) surviving Houses in Westeros should be allowed to run, should they become part of Volantis.
For example, I am playing as Aegon, son of Rhaegar and Lyanna, who is on the run with Dany and we are in Volantis. We are a part of the Volantene court, yet despite us being known Targaryens, and thus definitely Old Blood, we cannot attempt to run to become Triarch. This is even after I have become a Dragonlord, something the Volantene would absolutely actively court. This seems... odd.
If a way to integrate into Volantene politics was available for these houses (again, they must become Volantene, as in be a part of the Volantene Court) that would be great. Also a means to convert to the Valyrian religion and attempt to spread it. I'm sure the Old Blood isn't exactly happy that the religion of R'hllor has taken over their city outside of the Black Gates... Though I'll take even just the political aspect if nothing else.
On another note, it would be nice if Targs could make their own "Kings Guard" even while not actually ruling Westeros, as long as they hold the claim. Like Dany sort of does with Barristan.
There could also potentially be an event, one where the Targs-in-exile decide to "Return to their Old Ways" because obviously turning to The Seven ruined them - their dragonlords fought each other terribly, then they lost their dragons, then their throne, and perhaps they decide that "This all happened because AtC turned against the true gods, the Fourteen."
Lastly, Viserys shouldn't get the madness trait until he sells his mother's crown. He was fine before then, it was losing that, the final link to his mother that broke him. Similar to how it was his capture in the war that broke Aerys. Basically, Targs can get madness immediately, but even those who don't get it right away, can develop it due to stress. Thus adding Rhaella's crown to the game and an event where he becomes mad after selling it off would be nice, perhaps even an event where you can attempt to find it - possibly curing his madness.
